Before your next office visit, log on to MommyImSick.com. Click on the button labeled “Patients Click Here” followed by the “New Patient” button on the subsequent page. Enter all required information. Information is placed on a secure server and encrypted for complete security. You will be required to supply a credit card which will be held on file. It will only be charged if you use the system to obtain culture related information from MommyImSick.com. There are no charges for signing up for MommyImSick.com and you are notified for any charges prior to them being applied. Your social security number is requested but not required to become a member. Should you supply it we will use it to assist us with proper identification if required.
Once this information is on file, all member doctors can have access to it, if you schedule an appointment with them. There are no charges for medical providers to join and many advantages. So, if your doctors have not already joined, request they do so.
